Andrzej Bala, age 64, passed away after a long illness on August 21, 2014. Husband of Teresa Chmielewski; father of Agnieszka; step-father of Derek and Mark. Visitation at Michalik Funeral Home, 1056 W. Chicago Ave., Chicago, IL 60642 on Sunday, August 24 from 2-6 p.m. Family and friends will gather for a Funeral Mass, Monday, August 25, at 10:30 a.m. at Holy Trinity Church, 1118 N. Noble, Chicago.
